Lily Hospice

Founded in 2022, Lily Home & Hospice is family owned and operated by a family medical doctor and nurse practitioner who want to provide extraordinary care to those at the end of life. We incorporate "Jean Watson's Theory of Human Caring" into the framework of our mission because we feel it aligns with the values and principles of hospice care. We have a culture that is very kind, supportive, and encouraging. We work hard to create solutions to daily challenges and we take pride in being part of a team that makes a positive impact.

Our mission at Lily Home & Hospice is to treat the whole person - mind, body & spirit - by providing compassionate, person-centered care to those in the final stages of an advanced illness. We strive to meet each individual's needs throughout their end of life journey. We emphasize quality of life rather than the duration; and involve family and caregivers to make the experience more inclusive rather than isolated. It is our belief that each person deserves a peaceful, comfortable and dignified transition. Hospice care is provided by a team of healthcare professionals: medical director, registered nurses, certified nursing assistants, social workers, chaplains, and volunteers. The team works together to provide physical, emotional, and spiritual support to the individual and their family.
