Who We Are
Here at Horizon Home Care & Hospice, our volunteers are happy, productive, and making a real difference. We couldn’t do what we do in hospice without the positive energy and skill-set that volunteers contribute to the agency. To join our growing team of volunteers, all you need to do is: 1. Make time to volunteer because it is worth it! Volunteers often share that when they start serving in hospice, whether it's behind-the-scenes or directly helping patients, they have found the "secret" to living. 2. Choose from one of the many volunteer roles that best matches your time and talent. Direct patient care and many other opportunities are available at multiple locations throughout southeastern Wisconsin. 3. Reach out to us! We're some of the nicest people you'll find. Let us get you started on this awesome, purposeful experience!
What We Do
Horizon Home Care & Hospice was established in 1990 and today serves as the second largest full service home care and hospice agency in Wisconsin. We are a local, non-profit organization sponsored by Froedtert and Columbia St. Mary's Hospitals. As a leader in southeastern Wisconsin, we serve patients in Milwaukee, Ozaukee, Waukesha, Washington, Racine and Dodge counties, providing holistic care to those in home, at skilled nursing facilities and at our In-Patient Units in Mequon and West Bend. In addition, we have the only free walk-in Grief Resource Center in the state of Wisconsin.
